- 3. Summarize your Order below. Then detach it along Cut Line. (Keep the top portion for your records.)
- 4. Prepare your SA-SE = Self Addressed Stamped Envelope [E2 w/ postage indicated at the end of the line]. Where "3" = $.87 stamp; "2" = $.63 stamp. Note the minimum envelope dimensions.
- 5. Make your Check (US Funds) or US Money Order payable to BizMin Assoc. for the amount previously circled.
